A water bearer in India had two large pots, each hung on each end of a pole which he carried across his neck. One of the pots had a crack in it, and while the other pot was perfect and always delivered a full portion of water at the end of the long walk from the stream to the master's house, the cracked pot arrived only half full.
For a full two years this went on daily, with the bearer delivering only one and a half pots full of water to his master's house. Of course, the perfect pot was proud of its accomplishments, perfect to the end for which it was made. But the poor cracked pot was ashamed of its own imperfection, and miserable that it was able to accomplish only half of what it had been made to do. After two years of what it perceived to be a bitter failure, it spoke to the water bearer one day by the stream.
"I am ashamed of myself, and I want to apologize to you." "Why?" asked the bearer. "What are you ashamed of?"
"I have been able, for these past two years, to deliver only half my load because this crack in my side causes water to leak out all the way back to your master's house. Because of my flaws, you have to do all of this work, and you don't get full value from your efforts," the pot said.
The water bearer felt sorry for the old cracked pot, and in his compassion he said, "As we return to the master's house, I want you to notice the beautiful flowers along the path."
Indeed, as they went up the hill, the old cracked pot took notice of the sun warming the beautiful wild flowers on the side of the path, and this cheered it some. But at the end of the trail, it still felt bad because it had leaked out half its load, and so again it apologized to the bearer for its failure.
The bearer said to the pot, "Did you notice that there were flowers only on your side of your path, but not on the other pot's side?
That's because I have always known about your flaw, and I took advantage of it. I planted flower seeds on your side of the path, and every day while we walk back from the stream, you've watered them. For two years I have been able to pick these beautiful flowers to decorate my master's table. Without you being just the way you are, he would not have this beauty to grace his house."

The following information is to help you, as a patient with kidney failure, understand the issues regarding surgery to have long-term dialysis access. If you are being referred to see a vascular surgeon for dialysis access, it is either because your kidneys are no longer working correctly, in which case you are already getting dialyzed, or your renal (kidney) function is so poor, that it is likely you will need dialysis in the near future.
Dialysis can be performed through two ways: 1) Hemodialysis and 2) Peritoneal dialysis. The following information refers to hemodialysis only. Hemodialysis means that the body is cleansed by a filter in the hemodialysis machine. To be successful, the blood must flow through the machine at a high rate.
Tunneled catheters are best thought of as a bridge to permanent hemodialysis access. This involves a catheter (small tube) that is placed into a vein in the neck. It is called a “tunneled catheter” because it is tunneled under the skin. Although this is useful, these catheters should only be used for a short time. The longer these catheters are in place, the higher the risk of infection and the vein being narrowed.
Permanent hemodialysis access can be either an arteriovenous fistula or graft.
Arteriovenous Fistula
The arteriovenous fistula is the most durable option because it usually lasts much longer than an ateriovenous graft does. The fistula is a connection of the vein to the artery directly. Your vein must be of good quality, meaning that it must be a certain size and character. We routinely obtain an ultrasound of the veins to give us a good idea of the best veins to use. After the operation is done, the vein needs to mature – that is, it needs to get bigger in size and thicker before it can be successfully used for dialysis.
There are different ways for a fistula to be created. It is important to know that sometimes the veins that we would normally use are damaged or clotted because of previous punctures or intravenous lines. In that case, we may be able to use a vein that runs on the inner side of the arm, called the basilic vein. If we do use this vein, it will require two operations. This is because the basilic vein runs deep under the skin. For it to be usable, we have to raise it closer to the skin surface. We prefer to do this in two operations.
In either case, the vein will need at least 8-12 weeks to get big and thick enough to be used for dialysis. For this reason, it is great if your nephrologist (kidney doctor) refers you to a vascular surgeon before you actually need dialysis. If your fistula is made early, it will hopefully be mature by the time you do need dialysis, and you would not need to have a tunneled catheter placed.

Arteriovenous Graft
If your veins are not of good quality, you may need to have an arteriovenous graft placed. This is a man-made (Teflon) graft connecting the artery to the vein. The advantage of this is that it can be used in as soon as two weeks. The disadvantage is a graft does not last as long as a fistula; half of these grafts will clot off in the first year. For this reason, we try to make a fistula whenever possible, since they last much longer.

Su vena debe ser de buena calidad, lo que significa que debe tener un cierto tamaño y características. Normalmente, hacemos una ecografía de las venas para tener una buena idea de cuáles son las mejores para ese fin. Después que se realiza la operación, la vena debe madurar, es decir, debe aumentar de tamaño y de grosor antes de que se pueda usar para diálisis con éxito.

En cualquier caso, necesitaremos al menos 8 a 12 semanas para que la vena sea lo suficientemente grande y gruesa como para usarla para diálisis. Por este motivo, es muy recomendable si su nefrólogo (médico de los riñones) lo deriva a un cirujano vascular antes de que realmente necesite diálisis. Si la fístula se hace pronto, probablemente estará madura para el momento en que necesite la diálisis, y no será necesario colocarle un catéter canalizado.

Injerto arteriovenoso

Su ventaja es que puede estar listo para su uso antes de dos semanas. La desventaja de un injerto es que no dura tanto como una fístula; la mitad de estos injertos se coagulan durante el primer año. Por este motivo, siempre que es posible procuramos hacer una fístula ya que duran mucho más.
Operación y recuperación
Realizamos esta operación bajo anestesia local con sedación en más del 90% de los casos. La operación habitualmente demora alrededor de 2 horas. Después de recuperarse en la sala de recuperación puede irse a casa. Le colocarán un vendaje transparente estéril en el lugar de la cirugía, el que puede retirar en casa después de 72 horas (3 días) y dejar el lugar de la cirugía sin cubrir. Puede ducharse normalmente con la incisión descubierta, ya que las suturas son absorbibles y no será necesario retirarlas. Deberá volver a la clínica para que lo controlen alrededor de 2 semanas después de la cirugía.
Posibles riesgos

Otro posible riesgo es que se produzca una infección en la piel, lo que en la mayoría de los casos se trata fácilmente con antibióticos. En casos aislados, un injerto se puede infectar meses o años más tarde y habrá que retirarlo. Este riesgo es mucho menor con una fístula que con un injerto.
Es importante que usted comprenda que el término acceso a diálisis permanente no es preciso, debido a que muchas veces se deberá someter a los pacientes con insuficiencia renal a una o más operaciones adicionales.
