What is this document?

The first fields request information about the EMPLOYER and below of an EMPLOYEE (name, date of birth, address). Below that, it asks for AFFILIATION (cadre, article 36, employé, ouvrier) and below that RADIATION (date de sortie, motif)

yes I understand, but what is the term giving you problems, the "radiation" part or the "prevoyance" part ? Like I said, it's two different things.

The translation you need is of the following phrase: "Demande individuelle d'affiliation / radiation prevoyance".
