An accident was the result, but how to translate those two words? In this context woul lack of foresight on the part of both drivers be adequate?

I think "lack of foresight" is a little too indifferent. Is it trying to put the point across that both drivers made a close-minded judgement on what they saw without seeing the whole picture? It is lack of foresight but without the judgemental and value connotations that the words "sesgos y creencias" carry.

Bias refers to an INNATE PREDISPOSITION to believe something, so in inappropriate to refer to an accident or an incident that is part of an accident.
References to subjectivity, the subjective opinion of the driver, what the driver believed or understood, would all be more appropriate language for what seems to be an accident/insurance report
Lack of foresiught is an entirely different question altogether.
