diffuse neoplasm in upper outer quadrant with unclear outline
C.S. ext = Cuadrante superior externo
I am a bit uncertain about the 'sin limites determinados' though.
I am not sure if they mean to say that the neoplasm's outlines are not clearly defined (which should be the case by definition, since they are dealing with a diffuse neoplasm), or if they have not made any attempt to define the extent of the neoplasm.
This latter option seems to be the case in your first example sentence, since they state that the size should be determined by ultrasound.

Discussion
I don't think the 'upper outer quadrant of the throat' makes a lot of sense, and the 'upper outer quadrant of the axilla' even less.
'Indeterminate margins' sounds good to me, although I find it a bit self-evident when talking about a diffuse neoplasm.
Just to clarify, where it comes up in the exploración - pecho, axilas y garganta C.S. ext - does it mean chest, axilla, throat and upper outer quadrant? or is the C.S. ext relating to the garganta? or is the y in the wrong place? (from my research it seems quadrant relates to the breast and buttock).
