You could also say disqualified instead of excluded.
I presume that the bidder has to state whether he has ever been deemed unfit to submit a bid, for whatever reason.

This is the kind of statement that in English is generally not a header, but an actual first person declaration, something along the lines of:
"I hereby declare/state that there is no reason for my being excluded/disqualified from tenders"
The header itself would be something more summary like Statement of Qualification or something along those lines, so it's hard to keep this from sounding wordy in English.
I presume that the bidder has to state whether he has ever been deemed unfit to submit a bid, for whatever reason.
