Hochgradige Stenose des kleinen IM und intermediäre Stenose der CX. 

I'm not sure what IM stands for here.

I am posting a separate answer since Latin diagnoses are almost never used in English, certainly not in this case.

Intermediate artery is a brach of left main artery that is found in some people.
