Il ne s’agit pas d’une « mutuelle par avenant »… La phrase se décompose ainsi :
« ayant déjà négocié » …… « par avenant » : donc les négociations avaient abouti à un avenant (additional clause / rider).
Les négociations ont porté d’une part :
- sur les « sujets relatifs…… » ‒> signature d’un accord
- et sur « la mutuelle » (mutual insurance organization / friendly society) ‒> signature d’un avenant.
Voilà, j’espère que les choses sont maintenant un peu plus claires.

Are you sure 'mutuelle par avenant' is a term? It's just it sounds to me like company X might have reached an agreement with employee representatives, through the avenant dated 19 December 2012, on mutual fund arrangements (or however else you would translate mutuelle in the context of your text...). I could be off the mark, though.
