scarred myocardium
What was called "Schwielenherz" in older publications about infants (see David's references) is mostly Keshan disease (cardiomyopathy secondary to Coxsackie virus infection in a baby suffering from selenium deficiency).
For a more "modern" use of Schwielenherz, the ICD lists "Schwielenherz" as one of the options of previous myocardial infarction (I25.29; see http://icdscout.de/ICD2010/I25).
There is no precise correspondence in the American ICD list (e.g. http://www.icd10data.com/ICD10CM/Codes/I00-I99/I20-I25/I25-/... ), just a mention of "scar, scarring" (bottom of page), that's all).

calloused heart
not an official term, but would be a good description of the atherosclerosis

: may be suitable more for a biblical than for a medical context ("They close up their callous hearts, and their mouths speak with arrogance – Ps 17:10")
