French term
décompte individualisé

Top up health insurance plan
for a (French) complementary health insurance plan, under the head : what treatments and costs are covered?
Tous les actes et frais courant sur la période de garantie ayant fait l'objet d'un remboursement et d'un décompte individualisé du régime de base de sécurité sociale...
décompte appears to be 'premium adjustment' in an insurance context, but I can't make sense of it in this context!
and how can anything be 'individual' in a national health insurance scheme?

"décompte individualisé" refers to [Tous les actes et frais courant sur la période de garantie] and is paired with "remboursement".
These costs have been reimbursed by the [régime de base de sécurité sociale] on the basis of itemised bills (broken down to individual items).

Note from asker:
of course it is! I was so determined to find an 'insurance specific' term, I had forgotten about this obvious solution! Thanks! |

: I think "statement" is better, but this is the right idea.

: I agree with "itemised", but not with "bill".
"Individualisé" means pertaining to the individual, not the individual items. "Statement" fits the bill ;-)

I did hesitate between the two, but samples I could find made me think of "individual items" // costs are always attributed to/charged for a named individual, "individual" as a person seems implied anyway.

itemised list of reimbursements made
The "décompte individuel" is used here to describe a list which sets out expenses reimbursed by the Sécurité Sociale. These lists are drawn up monthly where expenses have been incurred and reimbursed. They can be consulted online and are also sent by land mail to the individual with cover.
The "décompte" itemises consultations, tests, medications and so on which a patient has paid and which have been reimbursed. It is not a bill in itself. A "décompte" may in fact refer to a number of different bills presented as follows :
- cost of consultation with Dr X on [date] : XX euros
- cost of X-rays with Dr Y on [date] : XX euros
- cost of medication paid at Pharmacy X on [date] : XX euros.
The "décompte" sets out the amount of the expense, the amount covered by the Sécurité Sociale and the amount borne by the patient (and the fact that the information has been forwarded to the Complementary Health Insurance company, if the patient has one).

"Statement", thank you. That's the missing word!

Note from asker:
Thanks Nikki, you are right this is more precise. In fact, I translated it as 'itemised statement' which is how I usually translate it, when I have my head screwed on the right way! |
