I've got a certificate (not to translate!!! this is more a personal thing) that I mostly understand but this term sounds tricky and too close to Croatian to be sure:
Poklic vzdzevalca matere
If I get it right it is a profession (or a job) of the
a) mother's guardian
b) person who takes care of the mother
I should fill in the form but this I could not understand...
many, many thanks!

Power Of Attorney
Perhaps "Power of Attorney". Legal Guardian is a legal term used mostly when talking about the care of children under the age of 18 or 21.
Example sentence:
He has been granted Power of Attorney over his mother, this includes her finances and health issues.
Note from asker:
See my comment: cannot be since I need to write the DOB, address and profession of this vzdrzevalec... |

Discussion
The question refers to who was the person caring for/helping the deceased. From the legal aspect, a caregiver can be someone who is also a person's legal guardian, with the power of attorney, but not necessarily.
Unless legal context is specified, both "legal guardian" or "power of attorney" are incorrect translations. I would say the correct term to use is "caretaker" or "caregiver".
