I don't think this is Petição Inicial. It is found on its own, at the end of a Decision document, right before the date and signature:

"... obrigar-se a comparecer sempre que intimado.

Possibility . . .

I'm not sure about the entire context, but if it involves a law suit it could be personal injury . . .
