This is one of many telegram-style entries in a table of test results of a female patient with various problems, ranging from pityriasis to urinary problems and possible H1N1:

Mit großer Wahrscheinlichkeit die H1N1 bei **Umgebungsnachweis,** Fieber und Husten und Schlappheit.

Not sure what is meant here. It was confirmed in the environment??

Might be an option. There are references for 'evidence of community transmission', and community seems to be used more commonly than surroundings or environment. Also lots of hits for community prevalence.

proof/evidence/indication of ambient prevalence

Whether you use proof/evidence/indication depends on the degree of certainty. I think "ambient prevalence" would be an apt rendering especially for a disease (quite a few Google hits for the term).
