acute-on-chronic response case

There were no breakthrough or acute-on-chronic response case observed.
Can anybody explain this "acute-on-chronic", please? May it deals with acute and chronic hypersensititvity reactions?

Hello, MMUIr! My text deals with a clinical study of Eligard in patients with advanced prostate cancer. This preparation is used to decline testosterone levels. Chapter 5.2.3 summarizes the results of phase 1 studies. It contains the sentence:"There was no evidence of any acute or chronic systemic hypersensitivity responses in any subjects." In chapter 5.2.5 "Efficacy results of phase 3 studies" appeared the sentense I am asking about. I am not sure about hypersensitibity. It's just a guess.

acute-on-chronic response with depot leuprolide

This is not about systemic hypersensitivity which was part of the safety discussion in Phase I trials - see your quoted sentence above.

Regarding leuprolide / Eligard treatment of patients with advanced prostate cancer, breakthrough and acute-on-chronic response are discussed. These terms have different definitions:

1. breakthrough:

--> increase of testosterone level to >50 ng/dL


2. acute-on-chronic:

"exacerbation"

"breakthrough" - a first reaction (while on study treatment),
"acute-on-chronic" - exacerbation of a pre-existing process.

In my context breakthrough is a sharp increase in testosterone lever following its reduction after administration of the drug. And we may call it exacerbation. "Acute-on chronic" is something different.

In Turkish they seem to be expressing acute-on-chronic as "acute on chronic (back)ground"

Thank you for active discussion of my postings! :) But my text deals with cancer treatment. I am not sure that we may talk about "acute' case of cancer advancing to "chorinc" one. It's possible for infections but not for cancer (IMHO).

Types of Pain

Pain in the veterinary patient can be divided into two general catagories -- acute and chronic pain -- with a third category that combines the two: acute on chronic.

Acute Pain. This is the pain generated by a traumatic or medical event that happens suddenly in an otherwise pain-free patient. Examples of events that can cause acute pain are surgical procedures, automobile accidents and other traumatic accidents, and medical conditions such as inflammation of the pancreas.

Chronic Pain. Chronic pain is often caused by conditions such as arthritis or chronic intervertebral disc disease. This is a very common category of pain in our patients.

Acute on Chronic. Occasionally we will see patients that have experienced chronic pain with a sudden increase in the intensity of their discomfort. A dog with bone cancer that develops a sudden fracture, or a patient with a chronic knee injury followed by a full tear of a ligament are examples of such pain.
