individual's application for police clearance
Romain's Dictionary of Legal and Commercial Terms gives "police clearance" as another option, although "certificate of (good) conduct" is also fine. I think "individual" is sufficient for "Privatperson" — also given in Romain, and putting it in front eliminates the ambiguity.

Perosnal Application for a Certificate Of Conduct
During my ten years in Germany such a certificate was required in all kinds of situations and it didn't always reflect good behavior. You needed to have one (and to present it) no matter what it said. On ealways hoped that it would show good conduct, and if it did not, you could be sure of getting what you were seeking.
Consequently, I would definitely leave out the "good."

I think the \"persona\" take care of the \"private person,\" i.e., it shows that the application is not \"official,\" i.e., from an official agency.
