Quando o casal assina uma declaração afirmando ou confirmando que mantém ou mantiveram na época de algum evento um relacionamento equivalente a um casamento ou que eram casados por determinado período em que adquiriram bens, por exemplo. Concluí a partir da leitura dos modelos de documento.

Pode ser Declaração de relação conjugal contínua. Pelos modelos dos documentos é uma declaração de que o casal está casado. E que deve ser feita em certas situações, compra de imóveis, seguros, independente de haver ou não uma certidão de casamento.

Record means write down or keep it somehow in some storage means.
continuous means without breaking up.
affidavit means declaration made by the own person, usually in writing.
In Brazil, de facto or marriage are synonyms, most of the time, meaning in the discourse of the native. In Australia, de facto is informal marriage, not legal, and marriage is formal marriage, with a legal agreement.

I just re-thought and then remembered that they say 'casamento estavel' in Brazil when the person has a marriage without interruptions.

I have just remembered that I helped someone in Interpreting recently in Australia, a cop, and she used 'affidavit' to mean oral declaration, so that this is also possible.
I am taking away 'de proprio punho', which can only be used in the sense of written declaration, and I am swapping 'casamento' for 'uniao' as well as 'sem interrupcao' for 'estavel', just because I remembered local Portuguese and think this is more like what things should be. It all comes down to context, I suppose.
